How do these hinges attach to door?

The blank stainless steel rectangle where you would typically find attachment holes are simply a cover plate that snaps in place once you attached it to the jamb with the screws. Hope this helps.

Between your door and mounting surface, you should try to maintain 1/4 inch

how much weight can these hold?

I have them attached to solid Mahagony bathroom stall doors the weight of the door is about 100lbs and the operate just fine.
