is this movement made in Japan?

I don't believe it's even a true seiko movement, seiko's are stamp either Japan or Malaysia depending on coutry of origin. This looks to be a chinese knockoff.

Will this mechanism fit in a 38mm Casio case with slight modification?

So long as the case will accept the width of the movement and its height it should work. But, be sure that the case tube will allow the stem to enter the movement evenly from left to right and top to bottom...

Does this movement tale three hands? And will it fit a 41 mm case

It has three hands: hour, minute, and second. It's a direct replacement for the earlier 7S26 and 7S36 movements, so if your Seiko case, regardless of the size, had one of those originally, it will drop right in. Be aware that you may have to swap in the black movement spacer from your old movement, as the gray spacer is a bit thinner.

The crown stem absolutely would not come ended breaking off in the movement and I know the correct procedure to get the crown stem out it would not come out ended up jabbing my tool into my index finger and bleeding everywhere
