Q: Can you use with with the nipple guard?

Yes I used it with the Medela nipple shield, I would put the tube under the shield and tape it in place.

Q: Does the milk only come down the tube when the baby sucks or does it come out on its own?

It can come out on its own, but it is very slow since the tube is so small- so baby sucking does most the work

Q: why does this say "disposable"? I already have the medela bottles with me

This version is only meant for short term supplementation, say a week or so. The tubing won't hold up to dozens of washings and sterilizations and use. I've seen them come loose from the tip, tube get crushed from being turned "off" and sometimes the tube just breaks. Q: Does it work with the Gerber bottles?

I don't know if it *works* but the Gerber bottles do fit the cap properly.
