No, I put wd40 in it, worked great.

Can You use it upside down? Will the oil come out using it this way?

The oil is driven out by pressure from the hammer-strikes on the end opposite the fitting. It doesn't flow by gravity, so there is no upside down to worry about.

how this thing work, do you hook it up to an air compressor and it blows old hardened grease out?

follow directions--no air compressor is used--a light weight oil is used--sometimes just changing the fitting is the best thing to do-- and the first thing to try--some people like heating joint with a torch--but this works by tapping tool with hammer
