What is the diameter of this product please?

The Maximum diameter is Three and a half inches (3 1/2")

Can the foam be washed? it has a very unpleasant smell...

Yes, it is washable. Indeed, I would recommend regular washing.

Is there any difference between 4" and 5"?

Mine is the original, full sized version which supports across the lower back. I do not secure the cushion, merely position it before I set off driving.

How long does it last? I’ve had mine for about 7-8 months and my back pain has returned. Has it softened?

I don't think they soften as such, although mine has slightly gone out of shape, so I have to keep turning it around. You may be better with the higher density one, apparently they do a higher density.
