can this be used for over toilet and for the shower??

Yes it can, our only issue with it, is our son in law who uses it is tall and although the height is adjustable which is good the foot rests aren't.hope this helps.

How do you claim the vat relief please

The seller should legally quote selling price including VAT then ask you to certify your entitlement to vat relief and deduct from quoted price when you buy it.
