Does this control the temp of your hot water or just allow you to measure it ?

It is both the thermostat to tell your Evohome what the cylinder temperature is, and the relay to switch the hot water valve to open or close. Usually the valve opening actually drives the heat demand from the boiler.

Can this work on it's own? Recently invested in the netatmo to control my heating and the hot water is running on the own programmable timer.

No I don't think so. It's an add-on to the central heating unit to control your hot water remotely.
