how frequently should I do the cleaning?

Either when your machine tells you, or when the milk frother doesn't froth as well, or if you see hardened spots or lumps of milk on your frothing nozzle, or once a week if you make milky drinks - unless of course the machine is commercially used, in which case daily as part of your machine hygiene routine.

Is this safe to use on an external milk caraffe eg gaggia cordorno prestige.

It's a straightforward enzyme cleaner. I use it (at the suggested dilution of 25ml of cleaner to 500ml cold water) to clean everything that milk passes through it is stored in, carafe, riser tubes, lids, pipes. If your system doesn't include an automated clean for these then a short soak will do the trick. The product seems to work by denaturing the protein and fat in milk making it very easy to rinse any residue (including the hard to remove milk film that sometimes happens) off things with hot water
