can i use them in sand

Yes they're actually better in sand because they won't fog the water up at all. Just try to place them directly under plants.

How long do they last before having to add a new one?

That will depend on how heavily your tank is planted and how big your plants are. I have three large amazon swords and put one of theses in to the root system of the plant at least once every 6 months. I have a sand substrate but the plants do have a soil ball around their base . Ii hope this has helped
