Generally speaking do you wear this against the skin/groin or over underwear/boxer brief/brief?

Speaking from experience I can tell you ya can do either. Personally I wear just the jock itself under gym shorts or football pants. But know guys that west it over their boxers too. It works just as well that way. This is a great jockstrap.

what size should buy if my wast is 34

Depends on how tight you want the waistband to be. They stretch pretty far. Medium is 32-34, but a Large will be less tight on the waist, and is probably a truer fit to a 34 without being too constricting.
