    Missing PPS, FAST, & KPS scoring

It’s a handy dandy notebook but it misses several opportunities. Oppose to asking about pain twice and medication 3 times I would have appreciated an area to note ADLS, IADLS, Falls, education, chaplain/social work needs etc. probably won’t purchase again.
