what is the lowest outdoor operating temperature for heating mode?

Neither of the two answers had to do with the HEATING function. Try again.

Can the condenser be mounted in an attic or does it have to be mounted outdoors?

NO, the attic gets too hot in summertime.

Does the kit include the line set cover?

No. It does not. You will need to buy that separately

Reviews cite a range from 20-30 Amp breaker for the 12K/115V. Max fuse rating is 25. What's best outside (fusable quick disconnect) vs at breaker box?

I used a Eaton 60 amp, 2 pole non fusable , enclosed AC switch disconnect near the outside unit. I also used 6 ft outdoor "AC Whips" for the wiring outside made by Southwire. I ran indoor wire from the breaker to a junction box inside near the exit to the outside and joined the outdoor wire(AC whip) there as well. As using indoor wire outside is not up to code.
