Does a 3 or 5 gallon water bottle come with it?

No water bottle comes with it, but you can order one separately or use a home delivery service such as deerpark. If you are considering this water cooler, I highly recommend it.

What does the open and close plate on the top do?

We only have a switch in the back with a circle and a line.. No plate... Never used it....

Where are the temperature switches located? Can the hot water switch be left on continuously (24 hours a day) or should it be shut off for fire safety

There are no switches. Once you plug in you will hear the thermosstadtkick on. Water will be hot & cold in about 20 min. Once in a while the unit trips my switch on the wall but I just press the reset button & sometimes I don't to save on energy. Actually I mostly only let it run when I'm have a family gathering. As for myself I just use it at room temp. If i want hot water I use my cusinart single brew.
