Is this a physical copy?

The one I purchased is a physical disk which must be inserted into the playstation. They do have a digital version for the playstations that do not take discs though.

- What are the dimensions of this item?

We live in a 3-Dimensional world.

Is this game really over 150GB now??

With all the updates, pretty much. You will need a minimum of 100gb to install, but then there are many updates (optional) which increase the game size.
