Is it safe when keeping it in a car? Can I also throw in an ice pack for it to be even colder?

Great question! Some 12v plugs in certain cars will work with the car on only. Depending on the car though. If the plug works with the car off it could drain the battery quicker.  You can throw in an ice pack however it will change the ice pack to the temperature inside the fridge.

What is internal temp? looking for a space for my insulin, but it cannot be frozen.

Very good question and one that we can not answer.  Because the temperature inside a "thermo Electric" cooler is determined by the temperature of the area it is in.

When you think refrigerators, you think a normal refrigerator.  That has freon, a compressor, fans, and takes a lot of electricity and can get down to freezing, even in a garage thats over 120 degrees. These are not that.  These are Thermo Electric coolers.  Back when I first looked into these, years ago.  These could only create a 30 Degree difference.  For example. If it is in a room that is 80 degrees, then the inside of the cooler would be at 50 degrees.    Things freeze at 32 degrees, so let's say for safety that modern thermo electric coolers are up to a 40 degree difference now.  so to keep it above freezing you could not keep this in a room colder than 73 degrees.   The room is 73 degrees, then the inside of the cooler would be 33 degrees.  one degree above freezing, which would be safe.

Look, these aren't bad devices and I very much would trust them to keep my life saving medication in, but you need to understand how these things work.  you buy this to keep your medication in your car, in Florida, where the inside of the car can get to 130 degrees, that means the inside of the cooler is at 100 degrees and your meds are ruined.  To be fair, you are most likely thinking of using this at work, in an office thats kept at a constant temperature.  In that instance you will be fine as long as the office isn't colder than 73 degrees, which most offices aren't.  I think most companies keep their offices at around 78.

I will be traveling by plane and hotel doesnt have fridge, will this fit into small suitcase or backpack?
