Is the Pi on the back of the screen covered after it is installed or is it left exposed?

The back is exposed so that you can attach the Pi to it. That is what I did and it has worked well. It also includes small cables (HDMI and charging) if you opt to mount the Pi on the back. You also get two small speakers which are connected on the back. If you have a Pi 4 you have everything you need. If you have a Pi 3 (like me) you have have to email the manufacturer for #3 connector. In my case there was a card in box. I'd like to get the HDMI connector & USB connector for the Raspberry Pi 3B+3B. The card says there is no charge The (ELECROW Micro USB to USB A Adapter & HDMI Adapter). They were most helpful on that and shipped that immediately (I got it in two days). Everything worked in on mine: speakers, display, touch screeen.

would it works with smartphone desktop mode?

If your phone supports USB to HDMI it works fine. I used on my Samsung 9 with a USB to HDMI adapter and both video and sound were good.

Does this have a audio out? I will not be using it with a raspberry pie but as a simple secondary screen for inside my windows PC tower.

Yes. It comes with two small speakers you can install on the back, and there is also a 3.5mm audio jack on the back of the left side of the screen.
