I put them lengthwise, front to back, just make sure it’s fully inside of the diaper. Otherwise, your little one will wake up soaked.

What size diapers do these inserts fit inside? Is there a size/age range for these pads to work appropriately?

I have used them in sizes 3, 4, 5 and now 6. In the smaller sizes the insert stuck out of the top a little but fit fine in all sizes in my opinion.

Can you use these for cloth diapers like to lay on top of the liner?

I would not see why not, it is similar to a fluffy pad that does do wonders in keeping one from leaking. I use them for my mother who has dementia and she will pee through everything even two diapers at a time. I bought these and placed it in the first diaper and now it seems to absorb all the urine which is great. It saves me money on diaper as well as doing so much launder to wash sheets and clothes.
