I’m confused of what this does exactly. Does it pinch the end of the cigarette or something? Please explain

No, it does not "pinch" the end of the cigarette. The cigarette is placed in the hole just below the filter and when the black part on top is pressed the 2 needles inside make 2 holes in the cigarette. This allows air to mix with the smoke. Once you get used to using 2 holes, you then move up to making 4 holes in the cigarette, and eventually work up to 8 holes. The instruction manual has precise instructions for placement of the holes. This thing is a total RIP off you stick the filter in it and push down it makes a small hole in the filter. You can do the same thing with a pin!

No, you have to do it multiple times. I stopped using mine. Either your lips or your fingers cover to holes so I gave up.

Is it more useful to use this in conjunction with a nicotine patch or without?

I would talk to your doctor. While generally you shouldn’t smoke while using nicotine replacement therapies, some have found success cutting down while on it or using two in conjunction with each other. Again please consult your doctor before even using one replacement therapy.
