The aperture off-loads the area of pain beneath the big toe joint. These medical grade 1/4"foam pre-cut pads are designed to relieve pressure on the sesamoid bones (under big toe joint). They are very comfortable, flexible, and durable. Easy to shape with scissors. They also reduce friction and skin irritation. These pads are adhesive backed to make for easy application. How to apply: Apply to clean and dry foot. Position pad so that the cut-out is comfortably placed around the head of the 1st metatarsal. Remove the cushion at least 3 to 4 hours daily to allow the skin to breathe. Recommended Care: none. Precautions: Do not place product on an open wound. I have purchased at least 5 different products, been to 3 Dr's, and these have been more helpful than anything else. These have a lot of adhesive, and will leave sticky residue on your foot, baby wipes removes it pretty well. They do also flatten with wear, so they really are a one-use possibly two use item. I recommend these over the felt ones as they stay in place better. I also like that these have a corner folded back so the adhesive cover peels off easily compared to other ones, making them quick to put on. I now have custom orthotics that do basically exactly the same as these, but since I don't wear my shoes in the house I find myself still wearing these. ### ⭐⭐⭐ Quality control issues with removing adhesive backing. *by M***O on Reviewed in the United States on December 14, 2019* These are my favorite sesamoid pads but my most recent purchase has some quality control issues. About 40% it is hard to remove the adhesive sticker back. I either peel a thin layer off the sticker but not revealing the stickie part or pull too much and damage the foam pad. I have had large calluses on the ball of my big toe for many years and finally in my old age they became painful. Along with treatment my podiatrist recommended pads like this and the difference they make is spectacular. I once again walk without pain. I tried the reusable gel ones and they worked fine but I could not get them to last more than a day or two so I settled on these.
