Yes after applying this do I wash it off

Yes, I always do because my ophthalmologist told me that it's best to rinse after cleaning the lids, no matter which product you use. I often use this after a warm compress to wash away any gunk that has been expressed from my oil glands.

What is difference between this and the lotion?

Having not used the lotion, just from the word description, I would say one’s a gel and one’s a lotion.
