How do you know how tight to make it?

When u feel that you are stretched vertically. You will know.

How long do you have to use it before you feel relief?

I used it for 3 days and I felt relief.

How do I clean this? It has a terrible odor. I wiped it down with a wet cloth but it still smells bad.

I got rid of the bag it was in and let it air out for a few days and it seems to be less offensive. I also used a towel over it the first few days that made it tolerable. It works really well once you get past the odor.
