**Q: does it have  high and low setting, or  a single setting?**
A: Hello, the HIGH / LOW setting is handled by the controller circuit board, the heating element will heat accordingly with the input of electric current (ampere).
So the answer is a YES if your fireplace has two settings of heating such as 750W + 750W (total 1300W of heating).

A fan is required and necessary. Since this heating element equips a thermo protection, if no airflow for pushing heated air out, the temperature in the fireplace will go up too much and quickly, then the thermo protection will shut off the power.
