Two hand warmers come in a pack. They are activated by shaking them in the air. Consider immediately placing the other one in a zip lock bag and squeezing the excess air out. I have been successful doing this.

Do they work in the toes?

The toe warmers are smaller, but if you have extra room in the toes of your boots I see no reason for the band warmers not to work.
