Pics seem to show two different steamers: one sitting down in the pot, and one sitting on rim, above pot. Which?

This item is three pieces. The lid, the steamer insert and the pot. Weirdest steamer I have ever owned, but happily the last. As with all my Staub purchases, very satisfied.

Does it have the spike lid ?

Yes. All Staub with dark enameled interior have the spike lids.

What's the ‘steamer insert’ made of?

STAINLESS STEEL With a Thick RUBBER RING ( Towards the Bottom ) to it From Bashing Against the Cast Iron Pot LIP !!!
