Do these come with a black/space grey buckle and adapter?

Mine has a silver buckle and did not come with any adapter.

I know this sounds crazy, but make sure your trying to slide it out on the correct side. It won’t slide out on either end - just one.

What is the best way to clean a dirty band?

I don’t know, I haven’t had it long enough to get dirty. I would imagine just a damp cloth with maybe some leather cleaner.

Does this band cause itching to wrist?

I have had mine for about three weeks and it has not caused any itching . But I also only wear during the work hours of the day , or in spurts .
