Is this like an electric toothbrush with vibration or just a manual brush?

It's manual, the baby just chews on it.

What is it made of? Is it silicone?

It says that is made from silicone,for me feels more like rubber but we have it from months and is very strong as my son is biting it so hard sometimes that I am surprised that is not damaged.

Can you get replacement brushes?

No,they are just normal one time use toothbrushes,just the ring can be removed and replaced.

Do you use this brush with baby toothpaste or can use with boiled cooled water? My baby’s gums have opened but no teeth yet (23 week old).

I used it with toothpaste but I imagine it can be used with water too as a teething aid
