So will this type of thing not mess up the top of a door?

I really don't know, as the top of the door is much higher than I am and I can't see it. It hasn't moved since I started using it, some time ago, so my guess is that it doesn't, as this is not a heavy-type item. My only concern was that the door would close, and it does. Hope this helps.

Does it come with instructions?

No it does not come with instructions but it is very simple to use. You just put the hooks over a door and adjust the rope by tying a knot to make it shorter or longer per your needs and it is ready to use. I have enjoyed mine very much and would recommend this product.

My door is abut 1 1/2 inches wide, which would be better the metal bracket door attachment or the web strap door attachment? Thanks

Both door attachment go over the top of the door. The metal door bracket is plenty wide enough to fit your door. It really comes down to preference. The metal bracket can stay on the door when you are done with your exercise. This can be helpful if your injury makes it hard to put the pulley up each time.
